What The Form Collects
The current form asks for name, email, phone number, interest areas such as peptides, hormones, circadian rhythm, and health optimization, plus a short message.
The form is not for emergencies, urgent symptoms, file uploads, full medical records, or time-sensitive clinical decisions.
What Submission Means
